Memphis Mission Trip
Parents -- Please read and fill out this form alongside your participating students
Who: Any Connect student and friends going into the 9-12th Grade
When: June 9-14, 2019
We will leave Sunday June 9, at 10:00am. We will return to SCC Friday night (6/14/19) around 9:00pm.
Where: The Students will travel to Memphis, TN to work with a ministry called Street Reach.
Cost: The total cost of the trip is $340, which covers meals, lodging and transportation. Students will need to bring extra money for dinner on their free night, dinner on the way home, and for any other things they might “need†to buy.
You can bring a check when you turn in the form, or you may pay at scctulsa.com/give or use the SCC App and choose Student Missions.
SCC Connect will provide fundraising opportunities to help with some of the costs. The amount of discount applied to the trip will be based on the student’s participation.

Sleeping bag/ sheets & blanket & pillow (A bunk and mattress will be provided)
Towels and Shower shoes
Bible - Devotional material
Work clothes (clothes that you don’t mind getting soiled or maybe ruined)
T-shirts (NO TANK TOPS)
Shorts (modest length)
Jeans
Casual clothes
Comfortable, safe closed-toe shoes (Tennis shoe or sneakers)
Sandals to wear when off duty as a missionary (open toed shoes are NOT permitted on club site)
Appropriate sleeping attire (you will be required to be FULLY dressed when out of your
bunk rooms and in hallways).
Toiletries & Sun block lotion
Prescribed medications if any
Christian music/devotional guides

Medical Release Form
Be it known that, I, the undersigned parent or guardian of the student above named, do hereby give and grant unto any medical doctor or hospital my consent and authorization to rend such aid, treatment, or care to said student as in the judgment of said doctor or hospital, may be required, on an emergency basis, in the event said student should be injured or stricken ill while participating in this South Community Church Event.
It is hereby understood the consent and authorization hereby given and granted are continuing, and are intended by me to extend throughout said student’s involvement in this South Community Church Event.
It is further understood that any expense incurred will be paid for by insurance or the parent/guardian of the student. Payment of the expense is not a South Community Church responsibility.
Emergency care: If emergency service involving medical action or treatment is required and neither the parents nor guardians can be contacted, I hereby consent for the student named above to be given medical care selected by South Community Church.
It is hereby understood the consent and authorization hereby given and granted are continuing, and are intended by me to extend throughout said student’s involvement in this South Community Church Event.

Covenant of Conduct
I understand fully that I am representing God, South Community Church, and myself on this trip. I will conduct myself accordingly and will try my hardest to do the right thing. I give Mark full authority, if he observes me not doing this, to come to a rational decision on whether or not to have my parents/guardians come and get me.

Release of all Claims
In consideration of being on this trip sponsored by South Community Church/SCC Connect, we (I ) being 18 years of age or older, for ourselves (myself) and for and on behalf of my child or ward do hereby release, forever discharge and agree to indemnify and hold harmless, the church, its trustees, board members, employees, volunteers, and or agents from any and all liability, losses, claims, or demands for personal injury, sickness and/or death, as well as property damage and expenses, of any nature whatsoever which may be incurred or sustained by the undersigned and/or the child or ward that may occur when the undersigned or child or ward is on this trip and or event, arising in whole or in part out of the acts, negligent or otherwise, of the church, its trustees, board members, employees, volunteers and/or agents.
Furthermore, on my behalf, or on behalf of our (my) child or ward, we hereby assume all the risk of personal injury, sickness, death, property damage and expense as a result of my or our child or ward being on this trip or event.
The undersigned further hereby agrees to hold harmless and indemnify the church, its trustees, board members, employees, volunteers and/or agents for any liability, losses, claims or demands sustained by the church, its trustees, board members, employees, volunteers, and/or agents, as a result of the negligent and/or willful acts of the undersigned or the undersigned’s child or ward, including expenses incident thereto.
We (I ) are (am) the parents(s) or legal guardians(s) of this child or ward and we (I ) hereby grant our (my) permission for the child or ward to be given attention medically if need be for any incident that may occur on this trip or event.
Furthermore, on my behalf, or on behalf of our (my) child or ward, we hereby assume all the risk of personal injury, sickness, death, property damage and expense as a result of my or our child or ward being on this trip or event.
